RESOLUTION TO APPROVE CHANGE ORDER NO. 2 FROM
SCHRADER EXCAVATING & GRADING CO. FOR THE 2021
SANITARY SEWER & WATER MAIN EXTENSIONS PROJECT
WHEREAS, the City Council of Fairfax, Iowa has contracted with Schrader
Excavating & Grading Co. for the 2021 Sanitary Sewer & Water Main Extensions project; and
WHEREAS, Hall & Hall Engineers has recommetlded approval of Change Order #2 as
described below.
Additional grading along E. Southview Rd. ditch to improve drainage.
Original Contract Amount: $ 1,829,2Q3.65

NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of Fairfax,
Iowa, to approve Change Order No. 2 from Schrader Excavating & Grading Co. for an
additional $2,482.50 making the conh�act amount $1,838,686.15.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of Fairfax,
Iowa, that the Mayor and City Clerk/Treasurer are hei•eby authorized and directed to execute
said resolution.
Passed and approved this 8t1i day of Febz-uary, 2022.

ITEM #1: Additional grading along E. Southview Rd. ditch to improve drainage
The existing ditch bottom along the north side of E. Southview Rd., prior to water main installation, contained
high points and drained poorly. It was requested that additional grading work be done, upon completion of
the water main, to eliminate the high points and improve drainage. This was beyond the Contractor's
original scope of work to restore the ditch bottom to existing grade.
The additional grading work was completed in November of 2021.
Refer to attached ITC #4 for detailed descriptions of the additional work.
The attached Change Order Request submitted by the Contractor provides the price increase for completing
the additional work.
The total Change Order 2 amount is $2,482.50.
There will be no additional working days granted for these work changes.

This "Instruction to Contractor" (ITC) is sent for the purpose of providing direction to the
contractor regarding the above listed Project. If you feel this Instruction results in an increase or
decrease in work defined by the current Contractual Agreement, you SHALL NOT PROCEED
until a Change Order Form has been submitted and approved. If work proceeds prior to
authorization, this shall acknowledge "No Change" to the current Contract Sum or Contract Time.
This "Instruction to Contractor" (ITC) shall become part of the construction documents upon its
approvai.
The intent of this instruction to Contractor is to:
❑ Clarify Documents and or Material Selections
� Identify a change to the Scope of Work
� Request an Estimate for Owner's Review and Consideration
Instruction to Contractor:
Grade north ditch bottom along E. Southview Rd. to eliminate high points and provide better
drainage, starting in front of 600 E. Southview Rd. going east towards Fairfax Rd. Refer to
attached drawings. An as-built topo survey of this area was completed on 11/10/21, which the
profile is based on.
Ditch bottom was previously graded per the grading file that was based on existing grades prior to
water main installation.
Existing ditch bottom grade prior to water main installation contained a few high points in front of
the Schuetzle property. The City requested that improvements be made to eliminate these high
points for better drainage, which is beyond the Contractor's original scope of work of restoring the
ditch bottom to existing grade. This area contains 993 feet of ditch bottom that needs cut to
eliminate high points.
In front of the 600 and 620 E. Southview properties however, the existing ditch bottom prior to
water main installation did not contain high points. Cutting these high points out, as shown on the
attached drawings, shall be completed at no additional cost to the City. This area contains 199
feet of ditch bottom that needs cut to eliminate high points. As previously discussed, the berm
that was located on the backside of the ditch in front of these properties still needs to be
reconstructed. Soil cut from the ditch bottom can be used for this.
