RESOLUTION APPROVING FLEX ONE PLAN
WHEREAS, the City Council of Fairfax, Iowa wishes to provide its etnployees
with the opportunity to obtain benefits; and
WHEREAS, the City Council does currently have a benefit package that includes
health insurance, life insurance, short-term disability, and AD&D insurance, which the
City of Fairfax currently pays the premium cost for qualifying einployees and inakes the
health insurance available to qualifying esnployees' fainilies at the employee's cost; and
WHEREAS, the City of Council desires to offer a wider variety of benefit
programs to the employees at the employee's cost; and
WHEREAS, AFLAC, Colonial Life, and Globe Life Liberty National have all
subinitted the paperwork participate in Fairfax's F1eX One 125 Cafeteria Plan that will
allow for preiniums to be pre-tax for all qualifying benefit plans except the life insurance
coverage and any disability coverage that may be chosen by an employee.
NOW, THEREFORE, SE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, that the City of Fairfax does hereby revise their Flex One 125 Cafeteria
Plan to enter into the Payroll Account Acknowledgement Agreements with AFLAC,
Colonial Life, ai7d Globe Life Liberty National to participate in this plan as of June 1,
2022.
BE IT FURTHER RESOLVED, by the Fairfax City Council that the City of
Fairfax hereby agrees to allow AFLAC, Colonial Life, ai7d Globe Life Liberty National
to make a presentation to all elnployees about their benefit prograrns.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk are hereby authorized and directed to
execute this resolution.
