A RESOLUTION TO APPROVE A REPORTING POLICY
WHEREAS,the City of Fairfax,Iowa has a need to receive federal grants and other federal
monies; and
WHEREAS, the federal regulations require the City of Fairfax to have certain policies in
place, which include a Reporting Policy.
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of Fairfax,
Fairfax, Iowa,that they adopt the attached Reporting Policy; and
BE IT FURTHER RESOLVED,by the Fairfax City Council of the City of Fairfax,Iowa,
that the Mayor and City Clerk are hereby directed to certify this resolution of approval.

CITY OF FAIRFAX REPORTING POLICY
PURPOSE
The purpose of this Reporting Policy is to ensure that the City of Fairfax complies with Code of
Federal Regulations, 2 CPR, Section 200.113 that requires reporting of violations of federal
criminal law involving fraud, bribery, or gratuity potentially affecting a federal grant.
POLICY
City of Fairfax departments and/or employees are required to disclose, in writing and in a timely
manner, all violations of federal criminal law involving fraud, bribery, or gratuity potentially
affecting a federal award. This requirement applies to violations involving City of Fairfax, its
employees, and any sub-recipients of a federal grant.
If a City of Fairfax department or employee learns of a violation of federal criminal law involving
fraud, bribery, or gratuity potentially affecting a federal grant, the department or employee must
report the violation to the City of Fairfax Auditor. Reportable violations include not only those
violations concerning City of Fairfax or its employees, but also include violations relating to sub-
recipients of award monies.
The City of Fairfax Auditor is responsible for reporting the violation to the relevant federal agency
in writing and in a timely manner.
