Add 12" Gate Valve to Distribution Main. Original Contract Amount: $2,278,183.54 C.O.#1 (Previously Approved) $4,606.20 C.O.#2 (Previously Approved) $3,284.27 C.O.#3(Previously Approved) $2,505.31 C.O. #4 $2.850.83 Revised Contract Amount: $2,291,430.15 NOW,THEREFORE,BE IT RESOLVED,by the City Council of the City of Fairfax, Iowa, to approve Change Order No. 4 from Kleiman Construction for the 2020 Pump House & Site Improvements project for an additional $2,850.83 making the contract amount $2,291,430.15. BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and directed to execute said resolution. CHANGE ORDER NO. 4 PROJECT: 2020 Pump House&Site Improvements DATE: November 5,2020 CONTRACTOR: Kleiman Construction ORIGINAL CONTRACT AMOUNT: $2,278,183.54 CONTRACT COMPLETION DATE: April 30,2021 ITEM#1: Add 12"Gate Valve to Distribution Main The 12"distribution main entering the pump house did not originally contain a valve in close proximity to the pump house. If any of the process piping inside the pump house would require future maintenance,the 12" distribution main would have to be shut off at the nearest valve currently located 85 feet away from the pump house. Shutting the 12"distribution main off at the valve 85 feet away would result in a separate 4"main being shut off as well. To avoid shutting off the 4"main while the 12"distribution main is shut off,an additional valve is required closer to the pump house. This additional 12"gate valve will be installed inside the pump house immediately after the point of entry of the distribution main. The attached Change Order Request submitted by the Contractor provides the price increase for furnishing and installing an additional 12"gate valve and the resulting pipe modifications required to fit the valve. The total Change Order 4 amount is$2,850.83. There will be no additional working days granted for these work changes.
