ELIOIBLITY FOR FULL TIME EMPLOYEE
HEALTH INSURANCE BENEFITS
WHEREAS,the City of Fairfax,Iowa is setting forth a decision on the requirement of
the amount of hours an employee has to work to be classified as a full time employee to receive
health insurance benefits,
WHEREAS,the new hire provisions must be met---Thirty Days After Hire Date--the
employee's coverage is effective the first of the month following completion of a one-month
period. (Examples: a Nov. 29 new hire is effective Jan. l; a Dec. l new hire is effective Jan. 1; a
Dec. 13 new hire is effective Feb. 1 New hires on the first of the month must wait for a one-
month period.),
WHEREAS, an employe: of the City of Fairfax,Iowa must work a minimum of
hours per week to be classified as a full time employee to be qualified to receive health insurance
benefits,
WHEREAS,the insurance coverage paid by the City of Fairfax,Iowa is Single Coverage
only,
WHEREAS,the City of Fairfax, Iowa does not offer health insurance benefits to part
time employees,
NOW THEREFORE BE IT RESOLVED,by the City Council of Fairfax,Iowa,that
the Mayor and City Clerk are hereby authorized and directed to execute said Resolution.
Passed and Approved on March 12,2002 by the following vote.
