A RESOLUTION REGARDING REVISION TO CHANGE ORDER NO. 5 TO
KLEIMAN CONSTRUCTION,INC. FOR THE CONSTRUCTION OF THE NEW
MECHANICAL WASTEWATER TREATMENT FACILITY,2006
WASTEWATER TREATMENT PLANT IMPROVEMENTS
WHEREAS, Resolution No. 2007-16 was passed on April 10, 2007, approving
Change Order No. 5 Kleiman Construction, Inc. to install extra loading dock ramp, as
shown HHE sketch dated 10-31-06; and
WHEREAS, Hall and Hall Engineers, Inc., has proposed a revision to Change
Order No. 5, Item 1 to allow waste hauling truck to back up to dumpster for removal of
material collected on mechanically cleaned screen in Blower/Screen Building.
BE IT RESOLVED,By the City Council of the City of Fairfax, Iowa,to approve
the revision to Change Order No. 5 for the construction of the new mechanical
wastewater treatment facility, 2006 Wastewater Treatment Plant Improvements, which
will result in an increase, as reflected in the attached schedule.
NOW, THEREFORE, BE IT FURTHER RESOLVED, By the City Council of
the City of Fairfax, Iowa, that the Mayor and Finance Administrator are hereby
authorized and directed to execute said Resolution.
