RESOLUTION APPROVING CHANGE ORDER NO. 2 FOR THE
WATER MAIN REPLACEMENT PROGRAM, PHASE II
IMPROVEMENTS
WHEREAS, The City Council of Fairfax, Iowa has contracted with Schrader
Excavating & Grading, Inc. for the Water Main Replacement Program, Phase II
Improvements, within the corporate limits; and
WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of
Change Order No. 2, for an increase to the contract amount of$4,466.00 as listed below:
C.O. #2, Item 1 $4,466.00
To add several culverts as a result of the ditch grading that has taken place on
Main Street. Eight inch culverts were used under the driveways to provide drainage
along the ditch line and ensure maximum coverage over the pipe. This item includes the
cost of supply of the 8" HDPE culverts only. Installation of these additional or
substituted culverts will be paid under Line Item#6 from the original bid documents.
BE IT RESOLVED, by the City Council of the City of Fairfax, Iowa, to approve
Change Order No. 2 for the Water Main Replacement Program, Phase II Improvements
which will result in an increase to the contract amount of $4,466.00 to make a revised
total construction contract cost amount of$410,764.86.
NOW, THEREFORE, BE IT FURTHER RESOLVED, by the Fairfax City
Council of the City of Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby
authorized and directed to execute said resolution.
