Home My WebLink AboutRESOLUTION NO. 2011-74 RESOLUTION NO. 2011-74 • RESOLUTION APPROVING CHANGE ORDER NO. 2 FOR THE WATER MAIN REPLACEMENT PROGRAM, PHASE III IMPROVEMENTS WHEREAS, The City Council of Fairfax, Iowa has contracted with Dave Schmitt Construction Company, Inc. for the Water Main Replacement Program, Phase III Improvements, within the corporate limits; and WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of Change Order No. 2, for an increase to the contract amount of$5,688.00 as listed below: ITEM#1: Adjustments to Water Main Alignment and Installation The existing water main is located approximately 4 feet east of where it was originally thought. It is therefore in the way of the proposed water main alignment. Engineering deemed it most feasible to move the proposed water main alignment 4 feet farther east. In doing so, 3 power poles and 2 trees become very close to the proposed water main trench and will require shoring and possible tree trimming. The method of pipe installation has also changed so that water main installation will begin at the north on Fairfax Road and continue south to a point beyond the existing water main. This change was made so that water service could be maintained to the residents on the east side of Fairfax Road while reducing the risk of a blowout at the existing water main bend near Fairfax Road and Linn Street during the proposed water main installation. In making this change, the water main will need to be tested in different segments. The cost of • this item is for the additional testing required for this segment, shoring and trimming of trees. Total cost for this item is $1,260.00, as reflected in the attached Change Order 2 Request#1 from Dave Schmitt Construction. ITEM#2: Plug Bushman Valve An additional valve was installed to the south of the T at the intersection of Fairfax Road and Linn Street. Daniel Bushman provided this valve, at no cost to the city to ease future connections to his property and properties to the south. This item is for a plug on the downstream end of this valve to ensure that the valve is not errantly opened with no water main connection present to the south. The total amount of this item is $350.00, as reflected in the attached Change Order 2 Request#2 from Dave Schmitt Construction. ITEM#3: Boring under Fairfax Road The decision was made to bore the water main under Fairfax Road at the intersection of Linn Street. The reason for this change was so that no Sealcoat pavement would have to be removed and replaced and Fairfax Road would not have to be shut down. The cost of this item is for the change of the water main material and additional equipment mobilization required to perform this work. The total amount of this item is $1,023.00, as reflected in the attached Change Order 2 Request #3 from Dave Schmitt Construction. Because of this work, the 6" Asphalt over 6" Granular Subbase needed to patch Fairfax Road (Line Item #43) will not be necessary. This is an approximately $2,400 bid item that should not be necessary as a result of this change. 1110 ITEM#4: Sitework Dave Schmitt Construction placed the spoils of the water main excavation along Linn Street to the south along the existing asphalt millings trail. This area was chosen instead of placing it on • Linn Street and the adjacent ditch slope due to the wetness of the excavated material and the narrowness of Linn Street. While this made it impossible to save the existing trail, it saved 3 weeks of road closure on Linn Street and preserved the sealcoat road and grass slope along the road, which are always problem areas on every project. The cost of this item is to replace the part of the disturbed trail that will not be constructed as PCC pavement in the spring as a part of the Murphy Ball Fields project. The total cost for this item is $3,020.00, as reflected in the attached Change Order 2 Request#4 from Dave Schmitt Construction. The total Change Order 2 amount is an addition of$5,688.00 Original Contract Amount: $266,714.80 C.O. #1 $50.00 C.O. #2 $5,688.00 Revised Contract Amount: $272,452.80 BE IT RESOLVED, by the City Council of the City of Fairfax, Iowa, to approve Change Order No. 2 for the Water Main Replacement Program, Phase III Improvements which will result in an increase to the contract amount of$5,688.00 to make a revised total construction contract cost amount of$272,452.80.
