RESOLUTION NO. 2011-84
RESOLUTION APPROVING CHANGE ORDER NO. 5 FOR THE
WATER MAIN REPLACEMENT PROGRAM, PHASE II
IMPROVEMENTS
WHEREAS, The City Council of Fairfax, Iowa has contracted with Schrader
Excavating & Grading, Inc. for the Water Main Replacement Program, Phase II Improvements,
within the corporate limits; and
WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of Change Order
No. 5, for an increase to the contract amount of $540.03 as listed below:
C.O. #5, Item 1 - Reimbursement for Insulation to Cover Shallow Areas in Water Main
The original contract and plan documents did not include the cutting of ditches along
Main Street. When these ditches were added at the request of the City of Fairfax, the plan depth
of the water main became too shallow to provide sufficient cover per the Cedar Rapids Metro
Specifications. These shallow areas were identified by the Engineer. The water main was
uncovered in these areas by Schrader Excavating and insulation was added. This Change Order
item reimburses Schrader Excavating for the cost of the insulation. Schrader Excavating
absorbed the cost of the installation. Please see the attached invoice from Construction
Materials, Inc. (Number 0107429) for verification of cost. The total amount for this Change
Order is $540.03.
BE IT RESOLVED, by the City Council of the City of Fairfax, Iowa, to approve
Change Order No. 5 for the Water Main Replacement Program, Phase II Improvements which
will result in an increase to the contract amount of $540.03 to make a revised total construction
contract cost amount of $ 414,334.89. .
NOW, THEREFORE, BE IT FURTHER RESOLVED, by the Fairfax City Council of
the City of Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and
directed to execute said resolution.
