RESOLUTION TO APPROVE CHANGE ORDER NO. 5 FROM
SEYDEL CONSTRUCTION FOR THE FAIRFAX PARK BUILDING PROJECT
WHEREAS, the City Council of Fairfax, Iowa has contracted with Seydel
Construction for the Fairfax Park Building Project, within the corporate limits; and
WHEREAS, Design Dynamics, Inc., has recommended approval of Change
Order No.5 from Seydel Construction. The current contract amount is $324,844.91. This
Change Order No. 5 is making the following listed change in the contract for a decrease
of$30.59. The final revised contract amount with Change Order No 5 is $324,814.32.
The following changes are needed due to Alliant Energy's needs being different
than those listed in the bid documents.
• Add for D1-3-1612-3 Sink Package +939.00
• Shipping +95.27
• GC OH&P +155.14
• Sub Total +1,189.41
• Deduct Previous Sink -644.00
• Deduct 2 Previous Faucets -576.00
• "Dotal Deduct -1,220.00
• NET TOTAL DEDUCT -30.59
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 5 from Seydel Construction for the Fairfax
Park Building Project.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and
directed to execute said resolution.

Attached is COR#5 for the three dip sink which with credit for the original sink as bid ends up
being a CREDIT to the owner of($ 30.59). The `new' sink is: Advance Tabco model DI-3-1612
3-compartment sink with 16"Wx20"Lx12"D bowls.
There may be some tweaking needed for the faucet. I'll be sending another email with the
cutsheets for the sink.
Also, could you send me an email `officially' telling me that the stainless steel counters are to be
fabricated and installed as bid? Hopefully that'll put that matter to bed.

the faucets are included in this package, if otherwise we will have to add the correct faucets on a
separate C.O.
