RESOLUTION TO APPROVE CHANGE ORDER NO. 3 FROM
DAVE SCHMITT CONSTRUCTION COMPANY, INC.
FOR THE FAIRFAX CITY PARK AND ATHLETIC FIELDS COMPLEX
WHEREAS, the City Council of Fairfax, Iowa has contracted with Dave Schmitt
Construction Company, Inc. for the Fairfax City Park and Athletic Fields Complex
Project, within the corporate limits; and
WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of
Change Order No. 3 from Dave Schmitt Construction Company, Inc. The current
contract amount is $1,334,226.87. This Change Order No. 3 is making the following
listed changes in the contract for an increase of$1,303.30. The revised contract amount
is $1,335,530.17.
Reduce Item No. 56 by 101 SY at $24.30/SY (2— 5" non-reinforced P.C.C. pads at 24' x
19' for each pad=> 24'XI9'X2EA=912SF/ 9SF/1SY= 101SY):
• Original quantity for Item No. 56 was 161 SY @ $24.30/SY= $3,912.30
• Remaining Qty of 5" non-reinforced P.C.C. is 60SY @ $24.30/SY= $1,458.00
• With this change, Item No. 56 is reduced by 101SY @$24.30/SY= ($2,454.30)
Add 6" Reinforced P.C.C. for the two bleacher pads @ 247"x 20' each pad:
• Unit price for 6" reinforced P.C.C. is $34.16/SY, which is from the $24.30
for the 5"non-reinforced misc. P.C.C. + $9.86/SY (additional paving
costs of$8.96/SY+10% mark-up).
• Area of the two pads (247" x 20' for each pad) => 24'7"X20'X2EA. = 983SF/
9SF/SY= 110SY
• 6"reinforced P.C.C. (110SY X $34.16/SY) _ $3,757.60
• Reduction in Item No. 56 = ($2,454.30)
Total Adjustment (with Change Order No. 3) Additional: $1,303.30
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 3 Dave Schmitt Construction Company, Inc.
for the Fairfax City Park and Athletic Fields Complex Project.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and
directed to execute said resolution.

A: Change the bleacher pads in size and revise them from 5" non-reinforced pavement to 6"
reinforced pavement(Item No. 56; Item Code 02700-01):
Upon the selection of the bleachers to be supplied with the park equipment from Boland Recreation, and
further review of these bleachers and their pad requirements, the size of the P.C.C. bleacher pads have
been increased from 24' x 19'to 247"x 20'. These pads have also been changed from 5"thick non-
reinforced P.C.C. to 6"thick reinforced P.C.C.:
Original Contract Amount: $1,329,859.87
Revised Contract Amount (After Change Order No. 2): $1,334,226.87
• Reduce Item No. 56 by 101 SY at $24.30/SY(2—5" non-reinforced
