RESOLUTION TO APPROVE CHANGE ORDER NO. 5 FROM
• DAVE SCHMITT CONSTRUCTION COMPANY, INC.
FOR THE FAIRFAX CITY PARK AND ATHLETIC FIELDS COMPLEX
WHEREAS, the City Council of Fairfax, Iowa has contracted with Dave Schmitt
Construction Company, Inc. for the Fairfax City Park and Athletic Fields Complex
Project, within the corporate limits; and
WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of
Change Order No. 5 from Dave Schmitt Construction Company, Inc. The current
contract amount is $1,335,530.17. This Change Order No. 5 is an addition to the contract
amount of $3,345.00, making the new contract amount $1,338,875.17 as described
below:
ITEM
A: Add a 2 foot diameter area intake with grate, approximately 91 LF of 6" dual wall
HDPE storm tile, connect to an existing storm line, and grade area to drain to this
area intake (Item No. NEW; Item Code NONE):
The area between the trail, the north ball field, the south dugout on this field and the bleacher pad
for this field is very flat and is ponding water along the south edge of the infield. An area intake
and 6" diameter tile are proposed to better drain this area and prevent water from ponding on the
ball field
Original Contract Amount: $1,329,859.87
• Revised Contract Amount (After Change Order No. 4): $1,335,530.17
Add 2 foot diameter area intake with open slotted grate and approximately
91 LF of 6" diameter dual wall HDPE storm tile, including connecting this tile
to an existing 10" HDPE storm line and grading area to drain to this intake:
o Lump sum for the area intake, lid, tile, connection to existing storm
line and grading area to drain to this intake - 1 LS @ $3,345.00 = $3,345.00
Total Adjustment (with Change Order No. 5): $3,345.00
Revised Contract Amount (after change Order No. 5): $1,338,875.17
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 5 Dave Schmitt Construction Company, Inc.
for the Fairfax City Park and Athletic Fields Complex Project.

BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and
directed to execute said resolution.
