RESOLUTION TO APPROVE CHANGE ORDER NO. 2 FROM
RICK.LEFS EXCAVATING, LTD. FOR THE PHASE 1 EAST
CEMETERY ROAD RECONSTRUCTION, DRAINAGE, STORM
SEWER, WATER MAIN, AND SANITARY SEWER
IMPROVEMENTS PROJECT
WHEREAS, the City Council of Fairfax, Iowa has contracted with Ricklefs
Excavating, Ltd. for the Phase 1 East Cemetery Road Reconstruction, Drainage, Storm
Sewer, Water Main, and Sanitary Sewer Improvements Project, within the corporate
limits and
WHEREAS, Nall, and Nall Engineers, Inc., has recommended, approval of
Change Order No. 2 from Ricklefs Excavating, Ltd, the current contract: amount is
$1,317,174.10 with an original contract completion date of 80 working days from August
14, 2013.
GENERAL DESCRIPTION OF CHANGE ORDER:
The Contractor had a delay due to an issue with the size of a trash basket to be installed
within the lift station. This issue has delayed the critical path of completing the lift station
and the dual 36" RCP storm sewer. This change order is to revise the number of working
days and milestone incentives per a request from the Contractor.
CHANGES 1N' SCHEDULE:
Milestone No.. 2
Int order to obtain incentive for meeting the Milestone No. 2 date of December 18, 2013,
the contractor was expected to complete all work (except seeding)including the dual 36"
RCP storm sewer. The change in contract Milestone No. 2 is to allow incentive if all
work except the 36" RCP storm sewer and seeding is complete. In order to obtain
Milestone No. 2 incentive of$300 per calendar day (with a maximum of$3,000), the
Contractor shall have the dual 36"RCP storm sewer complete by January 10, 2013.
Working Days
Contractor will be given.. seven (7) additional working days to complete the project for a
total,working day count of 87 days.
CHANGES IN CONTRACT AMOUNT:
There is no change in the contract amount associated with this Change Order.
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 2 from Ricklefs Excavating, Ltd. for the
Phase I East Cemetery Road Reconstruction, Drainage, Storm Sewer, Water Main, and
Sanitary Sewer Improvements Project.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and
directed to execute said resolution.
