RESOLUTION APPROVING CHANGE ORDER#6 FOR THE FAIRFAX TRAIL
PROJECT ALONG EAST SIDE OF UNNAMED TRIBUTARY TO PRAIRIE
CREEK,IDOT PROJECT NUMBER STP-E-2462(601) FROM TAYLOR
CONSTRUCTION,INC.
WHEREAS, the City Council of Fairfax, Iowa has contracted with Taylor
Construction, Inc. for the Fairfax Trail along East Side of Unnamed Tributary to Prairie
Creek between East Cemetery Road and Front Street in the City of Fairfax; and
W CI.EaREAS, Hall and Hall. Engineers, Inc., has recornrn.ended approval of
Change Order No. 6 from Taylor Construction, Inc. The current contract amount is
$800,128.00. This Change Order No. 6 is an addition to the contract amount of
$2.484.00, making the new contract amount $802.612.00 as described below:
GENERAL DESCRIPTION OF CHANGE ORDER:
Extend the steel piles, HP 10X57, (all 5) for the north pier and weld the HP 10X57 steel
pile extensions to the existing(design) steel piles, HP 10X57—5 each. The steel piles
did not reach the required bearing capacities at the design depth, so each pier was driven
additional depth to reach the required bearing capacity.

NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 6 from Taylor Construction, Inc. for the
Fairfax Trail along East Side of Unnamed Tributary to Prairie Creep between East.
Cemetery Road and Front Street in the City of Fairfax.

And weld the HP 10X57 steel pile extensions to the existing(design)steel piles,HP 10X57---5 each.
There is no change in working days due to this change order.
B-Reason for change or extra work:
The steel piles, HP 10X57,(all 5)for the north pier did not reach the required bearing capacities at the design depth,so each pier was driven
additional depth to reach the required bearing capacity. As a result,each steel pile,HP 10X.57 for the north pier needs to he extended to meet
the design elevation and these extensions welded to the existing(design)steel piles,HP 1 OX57,
There is no change in working days due to this change order.
