RESOLUTION TO APPROVE CHANGE ORDER NO. 1 FROM
RATHJEE CONSTRUCTION, CO. FOR THEPHASE 2 EAST CEMETERY ROAD
STREET RECONSTRUCTION, TRAIL,AND DRAINAGE IMPROVEMENTS
PROJECT IN THE CITY OF FAIRFAX,LINN COUNTY,IOWA
WHEREAS, the City Council of Fairfax, Iowa has contracted with Rathje
Construction Co. for the Phase 2 East Cemetery Road Street Reconstruction, Trial, and
Drainage Improvements Project within the corporate limits; and.
WHEREAS, Hall and. Hall Engineers, Inc., has rccoznmended approval of
Change Order#1 t'or a contract increase of$1,173.34, as described below:
CHANGES IN SCHEDULE: Done at this time.
CHANGES IN CONTRACT AMOUNT:
ITEM #1.1: ADD NEW BID —PCC Trail, 7"
There was a concern expressed by City Maintenance staff that the trail
pavement thickness on the south side of the road would need to be
thicker to accommodate possible vehicular traffic. This bid item is to
increase the trail pavement thickness on the south side of the road from
6" to 7" thick. This item is to be measured on an area basis and paid at
$2.911SY (in addition to the unit cost of Bid Item No. 67), Total
estimated increase in cost $797.34..
ITEM #1.2: ADD NEW BID—Additional Traffic Control Sign
There was concern expressed by Royale Pools that customers did not
know how to access their business if they turned on to East Cemetery
Road off Highway 151. An additional sign was added stating "Royale
Pools Open, Access From East End." This item is a Lump Sum item in
the amount of$200.00.
ITEM #1.3: ADD NEW BID Additional Staging at Commercial Entrance
As discussed in a Neighborhood Meeting prior to the project, the
pavement in front of Royale Storage Garages was to be installed in one
stage closing access for approximately one week. It was later
requested by the property owner to stage the construction such that the
entrance would be open at all times requiring the drive be poured half at
a time. This iitem is a Lump Sum item in the amount of$500.00 due to
the additional effort and time required for constructing the drive one half
at a time.
ITEM #1.4: ADD NEW IBID— Removal of Headwall and Culvert at Pace Supplies
Easterly Derive
The original plan was to keep the headwall and culvert in place at the
easterly drive of face Supplies. After removing the gravel and
inspecting the pipe, there was a mixture of different pipe materials
(HDPE and RCP) in poor condition. It was determined the entire 60'
long culvert would need to be removed and replaced. This also
required removal of the headwall which was not in the original bid. This
item is a Lump Sum item in the amount of$1,176,00.
ITEM #1.5: CREDIT- Pollution Insurance
At the time of the bid, an additional Pollution Insurance was specified.
This insurance was not needed and it was included in the Contractor's
mobilization costs. It is a credit to the project in the amount of$1,500.

Fairfax, Iowa, to approve Change Order No. 1. from Rathje Construction Co. for the
Phase 2 East Cemetery Road Street Reconstruction, Trial, and Drainage lm�provmnents
Project for an increase of$1,173.34, making the new contract amount $974,440.60.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and
directed to execute said resolution.
