HomeMy WebLinkAboutRESOLUTION NO. 2014-76 RESOLUTION NO. 2014-76 RESOLUTION APPROVING CHANGE ORDER 48 FOR THE FAIRFAX TRAIL PROJECT ALONG EAST SIDE OF UNNAMED TRIBUTARY TO PRAIRIE CREEK, IDOT PROJECT NUMBER STP-E-2462(601) FROM TAYLOR CONSTRUCTION,INC. WHEREAS, the City Council of Fairfax, Iowa has contracted with Taylor Construction, Inc. for the Fairfax Trail along East Side of Unnamed Tributary to Prairie Creek between East Cemetery Road and Front Street in the City of Fairfax; and WHEREAS, Hall and Hall Engineers, Inc., has recommended. approval of Change Order No. 8 from Taylor Construction, Inc, The current contract amount is $804,712.00. This Change: Order No. 8 is an addition to the contract amount of $50,507.50, making the new contract amount$855,219.50 as described below: GENERAL DESCRIPTION OF CHANGE ORDER: Flood Damage Repairs from the June 29'h and 30th Storm Events — 1.) Sediment and debris removal in and along the trail subgrade. This may include the need to import some suitable fill material to install in areas that may have been washed away in and adjacent to the trail subgrade. 2.) Clean the sediment from the two intakes and storm sewers. 3.) Repairing the washout area. along the cast embankment of the stream, approximately from Station 5+50 / 6+00 to Station 9+00 / 9+50. The stream channel has moved with this flood, and the scouring along the eroded embankment has caused the stream to sit right along this eroded embankment (Which is now very close to the proposed trail). This stream will need to be relocated to its original location (horizontally and vertically), and the scoured portion along the washout will have to either be de-watered and filled with imported clay material, or the water left in place and the scoured portion filled with shot rock and filler stone. Imported material would then be placed on this top of this, my thought is that this would be installed at a 6:1 side slope from the edge of the existing stream location back to the exiting grades pre-flood, and then armored with revetment per the original plans, CHANGES IN SCHEDULE: 5 additional working days (from 100 to 105 total working days). CHANGES IN CONTRACT AMOUNT: Change No, 7011 - 0120: 2102-2710070 Excavation, Class 10, Roadway and Borrow $5,688.00 Change No. 7012 - 0040: 2105-8425005 Topsoil, Furnish and Spread $1,210.00 Change No. 7013 - 0250: 2507-6800061 Revetment, Class E $9,240.00 Change No. 7014 - 0420: 2602-0000306 Perimeter and Slope Sediment Control Device, 6" Diameter $220.00 Chanve No. 9004—Clean Sediment from Intakes and 18"Diameter RCP Storm Sewer, Dispose of Off-site $770.00 Change No. 8005 —Macadam Stone, Used to Backfill The Scour Area in the Embankment Washout $10,367.50 Change No. 8006—Class E Revetment, Remove And Reuse Previously Placed Revetment $792,00 Change No, 8007—Remove Sediment and Debris from the Trail Subgrade and the Adjacent Trail Ditches, and Dispose of Off-site $17-,220.00 TOTAL: $50,507.50 Contract Amount Prior to Change Order No. 8: $804,712.00 Total Change in Contract Amount with Change Order No. 8: +50,507.50 Contract Amount after Change Order No. 8: $855,219.50 NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of Fairfax, Iowa, to approve Change Order No. 8 from Taylor Construction, Inc. for the Fairfax Trail along East Side of Unnamed Tributary to Prairie Crock between East Cemetery Road and Front Street in the City of Fairfax. BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of Fairfax, Iowa, that the Mayor and City Clerk/Treasu-rcr are hereby authorized and directed to execute said resolution.
