RE SOLUTION TO APPROVE CHANGE ORDER NO. 3 FROM,
RATHJE CONSTRUCTION,CO. FOR THE PHASE 2 EAST CEMETERY ROAD
STREET RECONSTRUCTION,TRAIL,AND DRAINAGE IMPROVEMENTS
PROJECT IN THE CITY OF FAIRFAX,LINN COUNTY,IOWA
WHEREAS, the City Council of Fairfax, Iowa has contracted with Rathje
Construction Co. for the Phase 2 East Cemetery Road Street Reconstruction, Trial, and
Drainage Improvements Project within the corporate limits; and
WHEREAS, Hall and Hall, Engineers, Inc., has recommended, approval of
Change Order#3 for a contract increase of$670.00, as described below:

ITEM #11: ADD NEW BID—Clean Out 24" Storm Sewer(June 30, 2014 Flood
Related)
After the June 30, 2014 Flood event there was a large amount of sediment
deposited in the storm sewer recently constructed which discharged into
the tributary that flows to Prairie Creek. Said tributary showed considerable
erosion and sediment damage after a greater than 100-year storm event
occurred. This item is a Lump Sum item in the amount of$670.

Phase 2 East Cemetery Road Street Reconstruction, Trial, and Drainage Improvements
Project for an increase of$670.00, making the new contract amount$975,390.80.

BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerl.Areasurer are hereby authorized and
