FOR THE PHASE 2 EAST CEMETERY ROAD STREET RECONSTRUCTION, TRAIL,AND DRAINAGE IMPROVEMENTS PROJECT IN THIS CITY OF FAIRFAX, LINN COUNTY,IOWA WHEREAS, the City Council of Fairfax, Iowa has contracted with Rathje Construction Co. for the Phase 2 East Cemetery Road Street Reconstruction, Trial, and Drainage Improvements Project within the corporate limits; and WHEREAS, Hall and Hall. Engineers, Inc., has recoinmended approval of Change Order#4 for a contract decrease of$6,748.00, as described below: CHANGES IN SCHEDULE: Due to the need to final seed some remaining areas in spring 2015, the Contractor agrees to complete any necessary final seeding by May 18, 2015, Otherwise the City may complete the seeding by other means. CHANGES IN CONTRACT AMOUNT: ITEM #4.1 ADD NEW BID ITEM Retaining Wall Design Only The improvement plans included construction of a retaining wall at the southwest corner of the project. Upon further review and coordination with the property owner, it has been decided to eliminate the wall and place additional fill at the location. This option is less costly and eliminates future maintenance needs of a retaining wall. The retaining wall quantity of 160 SF included in the bid items will be removed in its entirety. The Contractor incurred a cost associated with a structural design for the retaining wall as part of the submittal review process. This item will be measured as a Lump Sum in the amount of$1,950 to compensate for the design of the wall. ITEM #4.2: REVISE UNIT PRICE—Asphalt Cement Concrete Pavement, 8" The original improvement plans included an ACC transition from the east end of the proposed East Cemetery Road ICC pavement to the existing seal coat pavement. The Contractor requested paving this as PCC leaving the remaining 20-foot to be an ACC transition. The change in ACC quantity was a reduction greater than 25% and Cedar Rapids Metro Specifications allows the Contractor to negotiate a change of unit price due to the decrease in quantity. This item is to be paid by the Ton with a unit price of $8.00 per ton. Even with the increase in unit price there was an overall cost savings by changing to PCC pavement. ADJUSTMENT OF PLAN QUANTITY: Portland Cement Concrete Driveway, 6 In. on 4" Granular Subbase At the request of the property owner, the driveway at 85 East Cemetery Road was removed from the construction contract to be paved by others.
