RESOLUTION APPROVING CHANGE ORDER##12 FOR THE FAIRFAX
TRAIL PROJECT ALONG EAST SIDE OF UNNAMED TRIBUTARY TO
PRAIRIE CREEK,IDOT PROJECT NUMBER STP-E-2462(601) FROM
TAYLOR CONSTRUCTION,INC.
WHEREAS, the City Council of Fairfax, Iowa has contracted with Taylor
Construction, Inc. for the Fairfax Trail along East Side of Unnamed Tributary to Prairie
Creek between East Cemetery Road.and Front Street in the City of Fairfax; and
WHEREAS, Hall and Hall Engineers, Inc., has recommended approval. of
Change Order 'No. 12 from Taylor Construction, Inc. The current contract amount is
$859,276.33. This Change Order No. 12 is an addition to the contract amount of
$2,790.00, making the new contract amount $862,066.33 as described below:
A-Description of change to be made or extra work to be done:
7017—Addition to the Contract for Reseeding of Rural Seeding and Fertilizing (Item Code 2601-2636043)of disturbed areas where the
original seed has been washed away due to flooding. All seeding shall be completed per the GS-12003 Specifications, Effective Gate of
October 15, 2013, Section 2601.03. The Seed Mixtures shall be per Section 2601.03.0.3 Rural Seeding of the GS-12003 Specifications,
Effective Date October 15,2013, The area for the Rural Seed Bed Preparation and Reseeding is estimated at approximately 0.5 Acre.
Reseeding shall be paid at the Contract Unit Price. Mobilization shall not be considered as part of this change order.
7018--Addition to the Contract for Reseeding of Urban Seeding and Fertilizing(Item Code 2601-2636044)of disturbed areas where the
original seed has been washed away due to flooding. All seeding shall be completed per the GS-1'2003 Specifications, Effective Cate of
October 15,2013, Section 2601.03, The Seed Mixtures shall be per Section 2601.03.C.4-Urban Seeding of the GS-12003 Specifications,
Effective[date October 15, 2013. The area for the Urban Seed Bed Preparation and Reseeding is estimated at approximately 0.3 Acre.
Reseeding shall be paid at the Contract Unit Price.
8010--Mobilizations, Erosion Control shall be applicable for the seeding contractor to stage movement of labor, equipment, materials and
other incidental items for completion of the reseeding work, per GS-12003, Section 2602. This item shall be paid per incident(1 EA)at the
unit price of$500,00 for each Mobilizations, Erosion Control, as specified in GS-12003, Section 2602.05.
There is no change in working days due to this change order. Because this work is in addition to the original contract work,working days will
not be considered for the Reseeding of the Rural and Urban Seed Mixtures, The Contractor shall complete the Reseeding within the first two
weeks of the normal fall seeding dates(August 10„2015 to August 21,2015),
B—Reason for change or extra work;
On the evening of June 11, approximately 2 weeks after the contractor had seeded the project site,the City of Fairfax received a substantial
rain event that was consistent with a 25 year storm event. Prairie Creek rose significantly,as did the unnamed tributary adjacent to the trail.
In the area where the unnamed creek turns south,this stream came out of its banks. These areas, as well as a portion of the ditch east of the
trail and adjacent to the farm field had the seeding washed away.
Contract Amount Prior to Change Order No. 12: $859,276.33
Total Change in Contract Amount with Change Order No. 12: ..+-2,790.00
Contract Amount after Change Order No. 12: $862,066.33
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order 'No. 12 from Taylor Construction, Inc. for the
Fairfax Trail along East Side of Unnamed Tributary to Prairie Creek between East
Cemetery Road and Front Street in the City of Fairfax.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and
directed to execute said resolution.
