RESOLUTION TO APPROV_ ' CHANGE GE ORDER NO. z .Y RO
DAVE SC - MITT CONSTRUCTIONCOMPANY, INC,.
FOR THE KARAL'S DRAINAGE IMPROVEMENTS PROJECT
WHEREAS, the City Council of Fairfax, Iowa has contracted with Dave Schmitt
Construction Company, Inc. for the Karal's Drainage Improvements Project, within the
corporate limits; and
WHEREAS,Hall and Hall Engineers, Inc.,has recommended approval of Change
Order No. 2 from Dave Schmitt Construction Company, Inc. The current contract amount
is$98,964.50. This Change Order No. 2 is an addition to the contract amount of$1,833,25,
making the new contract amount $100,797.75 as described below:
ITEM##1: Installation of Area Intake, 'File Connection
In order to alleviate shallow slope/ponding concerns on the northern end of this project,
an area intake, and 6" drain file connection to a nearby standpipe is proposed to be
installed near the DOT ROW on Vance's property.
The total adjusted Change Order 2 amount is $1,833.25. See Dave Schmitt Construction
Co., Inc. Proposal dated August 27, 2015 (attached) for additional breakdown of costs for
this change.
Original Contract Amount: $106,327.00

Revised Contract Amount: $100,797.75
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 2 Dave Schmitt. Construction Company, Inc.
for the Karal's Drainage Improvements Project.
DE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa,that the Mayor and City Clerk/"Creasurer are hereby authorized Ind directed
to execute said resolution.
