RESOLUTION TO APPROVE CHANGE ORDER NO. 3 FROM �
DAVE SCHMITT CONSTRUCTION COMPANY, INC.
FOR THE KARAL'S, DRAINAGE IMPROVEMENTS PROJECT
WHEREAS, the City Council of Fairfax, Iowa has contracted with Dave Schmitt
Construction Company, Inc. for the Karal's Drainage Improvements Project, within the
corporate limits; and
WHEREAS,Hall and Nall Engineers, Inc.,has recommended approval of Change
Order No. 3 from Dave Schmitt Construction Company, Inc. The current contract amount
is $1.00,797.75. This Change Order No. 3 is an, addition to the contract amount of
$4,1397.00, malting the new contract amount $1.05,694.75 as described below:
ITEM#1: Reinforcing Steel for Concrete Flume
In order to alleviate heaving/differential settlement of the concrete flume through freeze
and thaw cycles, the City of Fairfax has requested that wire mesh be added to reinforce
the concrete flume. The type and quantity of wire mesh installed will be 5,900 SF of
W4-D4 @ 4".
The total adjusted Change Order 3 amount is $4,897.00. See Dave Schmitt Construction
Co., Inc. Proposal dated August 28, 2015 (attached) for additional breakdown of costs for
this change. The City has requested the heavier option(Dave Schmitt Invoice Item 92) as
shown on the attached proposal.

Revised Contract Amount: $105,694.75
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No, 3 Dave Schmitt Construction Company, Inc.
for the Karal's Drainage Improvements Project.
IBE IT FURTHER RESOLVED, 'by the Fairfax City Council of the C:,ity, of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and directed
to execute said resolution.
