HomeMy WebLinkAboutRESOLUTION NO. 2016-36 RESOLUTION NO. 2016-36 RESOLUTION TO APPROVE CHANGE ORDER NO. 1 FROM RATHJE CONSTRUCTION, CO. FOR THE PHASE 4 WATER MAIN AND VANDERBILT PAVEMENT RECONSTRUCTION PROJECT IN THE CITY OF FAIRFAX, LINN COUNTY, IOWA WHEREAS, the City Council of Fairfax, Iowa has contracted with Rathje Construction Co. for the Phase 4 Water Main and Vanderbilt Street Reconstruction Project within the corporate limits; and WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of Change Order#1 as described below: ORIGINAL CONTRACT AMOUNT: $ 752,644.32 ORIGINAL CONTRACT COMPLETION DATE: 85 Working Days ITEM #1: 15" RCP FES with Toe Wall ITEM #2: 5" PCC Erosion Protection Slab on 4" Stone Subbase ITEM #3: Negate RA-5 Intake (Line Item A1-8)from the Project Quantities The corrugated metal pipe uncovered on the north side of the Park Street/ Front Street Intersection was much deeper than originally anticipated. Due to this added depth, the original plan of improvement to add a new RA-5 Intake saddled over the existing pipe to accept the drainage pipe from the new circular grate intake on the southeast side of the intersection was no longer feasible. The most cost effective change is to eliminate the proposed RA-5 intake, and run the new drainage pipe north from the grate intake and outlet the pipe in the grass slope north of Front Street. These changes will result in the addition of a Flared End Section and a subsequent erosion protection slab, resulting in line items 1 and 2. Please see revised plan sheet D.03, and Rathje Construction COR-1, attached. • Line item 1,"15" RCP FES with Toe Wall": 1 Unit @$1,680.00 • Line Item 2, "Y PCC Erosion Protection Slab (Reinforced with Wire Mesh Mat or Engineer-Approved Equivalent Rebar on 4" Stone Subbase": 33 SF @ $49.50 = $1,633.50. • Line Item 3, "Negate RA-5 Intake (Line Item A1-8) from the Project Quantities": 1 Unit @ $3,700.00 The total adjusted Change Order 1 amount is($386.50). Original Contract Amount: $ 752,644.32 C.O. #1, Item 1 ($386.50) Revised Contract Amount: $752,257.82 NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of Fairfax, Iowa, to approve Change Order No. 1 from Rathje Construction Co. for the Phase 4 Water Main and Vanderbilt Street Reconstruction Project for an additional $386.50 making the contract amount $752,257.82. BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and directed to execute said resolution.
