FOR THE UV DISINFECTION SYSTEM PROJECT IN THE CITY OF FAIRFAX,LINN COUNTY, IOWA WHEREAS, the City Council of Fairfax, Iowa has contracted with Ricklefs Excavating, Ltd. for the UV Disinfection System Project within the corporate limits; and WHEREAS,Hall and Hall Engineers, Inc., has recommended approval of Change Order#1 as described below: ORIGINAL CONTRACT AMOUNT: $ 243,500.00 ORIGINAL CONTRACT COMPLETION DATE: July 22, 2016 ITEM #1: Clarification to Enaqua UV Disinfection System Requirements This change is to ensure conformance with the Iowa DNR Construction Permit issued on 5/4/2016 (number 2016-0372S). This permit requires a clarification to the acceptable UV Disinfection System. The UV Disinfection System Supplier is to provide the City of Fairfax with a System capable of Disinfection Dosage of 30 mJ/cm2 based on the fastest system velocity, NOT assuming uniform velocity. This is an Iowa DNR—mandated calculation clarification, not a difference in requirements from the bid documents. No change has been made to the disinfection requirements form the bid documents, resulting in a zero-dollar change to the contract documents. Please see the attached documents, as approved by the IDNR. The total adjusted Change Order 1 amount is($0.00). Original Contract Amount: $243,500.00 C.O. #1 Item 1 ($0.00) Revised Contract Amount: $243,500.00 NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of Fairfax, Iowa, to approve Change Order No. 1 from Ricklefs Excavating, Ltd. for the UV Disinfection System Project for no additional costs keeping the contract amount $243,500.00. The Contractor is responsible for obtaining utility locations and potholing utility lines as needed. The following utility companies are in the area and have corresponded with the ENGINEER regarding the project. The Contractor shall include a full set of specifications and shop drawings for the proposed UV System as a part of the required bid documents package at the time of bid letting so that City personnel and the Iowa Department of Natural Resources can confirm the proposed system meets all permit requirements prior to the awarding of bids for this project.
