HomeMy WebLinkAboutRESOLUTION NO. 2016-44 RESOLUTION NO. 2016-44 RESOLUTION TO PROCEED WITH THE INITIAL PHASE OF THE GRADING AND EROSION CONTROL IMPROVEMENTS TO HEARTLAND HEIGHTS FIRST ADDITION IN THE CITY OF FAIRFAX, LINN COUNTY, IOWA WHEREAS, a Plan of Improvements dated May 23, 2016, has been submitted to the City of Fairfax, Iowa, from Iowa Lot Development Co. and Nick Bettis, PE., with HBK Engineering, L.L.C. for the Initial Phase of Site Grading and Erosion Control for the Heartland Heights First Addition in the City of Fairfax, Linn County, Iowa; and WHEREAS, Hall and Hall Engineers, Inc. has reviewed said Initial Phase of Site Grading and Erosion Control Plans for grading activities of said improvements in the Heartland Heights First Addition in the City of Fairfax, Linn County, Iowa and has recommended approval of this Initial Phase of Site Grading and Erosion Control Plan of Improvements with the following conditions: A. That prior to the City Council Considers any further public improvements to the Heartland Heights First Addition Plan of Improvements, that the following comments to the Initial Phase of Site Grading and Erosion Control Plans are addressed and resubmitted to the City of Fairfax: 1. It is my understanding that this is not the final grading plan of the Heartland Heights First Addition Improvements, and that Basin A will be expanded with approval from the Iowa DNR. Please Clearly label these grading plans as the Initial Grading Improvements for Heartland Heights First Addition, and add a note to these plans that these Plan of Improvements are the initial grading and erosion control plan of improvements, and that the final overall grading and erosion control plans for Heartland Heights First Addition will be submitted for review and approval upon the Iowa DNR's approval for the additional grading of Basin A. 2. On the Cover Sheet, please revise the Index of sheets to reflect only the General Notes and the Grading and Erosion Sheets. 3. On the General Notes Sheet, please remove the notes that do not pertain to the site grading, erosion and sediment control, and site restoration. Please also add the list of CR Standard Details to this sheet that only pertain to the site grading, erosion and sediment control, and site restoration for this site. Please make sure that all notes pertain to this project, and the City of Fairfax. 4. Clearly label all proposed contours at 5' intervals, including north of Beverly Road and south of Heartland Street. 5. There appears to be low points with the proposed contours plan where water will pond. Please address these, and label all low point and high point contours. 6. Please label Beverly Road within the proposed grading limits on Plan Sheet C5.1. 7. Please clean up the proposed contours. 8. On the Grading Plan Sheets, please show the location where the SWPPP/ NPDES Permit is being stored on-site. Please note what is being used to house this document, and who is the responsible person (Name and Contact Information) who is maintaining this document. 9. Please clearly identify and label the slopes for all side slopes that are at a 4:1 slope or steeper. No slopes steeper than 3:1 are acceptable, with 4:1 being the preferable maximum slope. 10. Remove services from the grading plan sheets, and clean-up the utilities so that they all have the same legible line type scales. 11. Please show and label (type and size) all existing and proposed easements on the grading plan sheets. 12. On Plan Sheet C5.0, how is Borrow Area 2 accessed? If a new stream crossing of the east-west creek is necessary, please show this, with the proposed type, size and length of culvert proposed. 13. What is the proposed outlet protection at the outlet of the existing culvert under 80th Street at the southeast corner of this Addition? What is the proposed outlet protection of the proposed culverts under Bridgewood Drive and Eagleview Drive? Please show these, and provide the design information for these. 14. Is there a proposed Culvert under Beverly Road at 801h Street? If so, please show this culvert, and the proposed outlet protection. 15. Please show the gas main through the grading limits on this site, and provide the approximate top of gas main elevations throughout the site grading limits. Please submit the grading plan to Greg Wildebour with Mid-American Energy and obtain his approval prior to grading over the existing gas main. Please copy me and the City of Fairfax with Mid-American Energy's Comments/Approval. 16. There appears to be a drainage swale located along the south property line of Heartland Heights First Addition. The proposed contours along this drainage swale do not appear to contain the drainage in this swale within the storm water drainage easement. Please provide a typical section of this swale and make sure that the proposed contours reflect this typical section. 17. The proposed drainage swale along the south property line of Heartland Heights First Addition shall be constructed and final graded and seeded as part of the plan of improvements. Please reflect this in a note. 18. On Plan Sheet C5.0, The existing wetlands and the north-south and east-west streams are shown with 50' and 100' protective buffers respectively. It is my understanding that the wetlands are currently grassed and not farmed. Is this correct? Is the area around the north, west and south sides of this wetland area planned to be farmed? If not, then this area should be grassed as well. Please show the existing grassed areas of the wetlands and along the two streams, and the proposed grass buffer areas around the wetlands and streams as well. 19. As part of the Site Restoration Plan, please show the farming limits along the exterior of the site, and the existing ground cover(with type of cover) to be maintained on the site and adjacent to this site. Please also show the proposed ground cover and note the type of cover and whether or not this is a temporary ground cover of permanent ground cover. The permanent drainage swale along the south property line, as well as the Beverly Road right-of-way should be final graded with topsoil and seeded with permanent ground cover. 20. Are the stockpile and construction entrance locations as shown on the grading plan consistent with what is currently being done on-site by the contractor? Please coordinate with the contractor on this and revise accordingly. 21. Please provide 4 sediment traps (per SUDAS 7E-13). These should be located along the permanent and temporary swales at the following locations: North side of Lot 183, the south side of Lot 88, the south side of Lots 96 & 97, and the south side of Lots 11 & 12. 22, On Plan Sheet C1.2 —General Notes, under Grading Notes No. 12, please list the Lot Numbers that this note pertains to. 23. Under the Grading Notes, please add a note that all organic materials shall be removed from the Public R.O.W.'s and that fill placed within the Public R.O.W. shall consist of approved materials which are free of organic matter and debris. That fill within the R.O.W. shall be placed in loose lifts of 9" or less, and that fill within the public R.O.W. that is greater than 18" shall be tested per the Cedar Rapids Metropolitan Area Standards Specifications, Section 01110, 3.02—Geotechnical Testing. Please provide the locations where these tests shall be required (Street names and stationing) and please have these test results submitted to the City of Fairfax and Hall and Hall Engineers, Inc. 24. Under the Surface Restoration Notes, No. 1, please revise this note to say that"All disturbed areas not paved or hard surfaced shall receive a minimum of 6"? of topsoil. Scarify areas...." The City only required a minimum of 4" of topsoil, but if the owner/ developer prefers 6", this is also acceptable. NOW, THEREFORE BE IT RESOLVED, that the City Council of Fairfax, Iowa, approves Initial Phase of Site Grading and Erosion Control Plan of Improvements for the Heartland Heights First Addition in the City of Fairfax, Linn County, Iowa with the conditions as listed above and submitted by Hall and Hall Engineers, Inc. BE IT FURTHER RESOLVED,by the City Council of Fairfax, Iowa,that the Mayor Pro Tempore and the City Clerk are hereby authorized and directed to execute said Resolution.
