FOR THE PHASE 4 WATER MAIN AND VANDERBILT PAVEMENT RECONSTRUCTION PROJECT IN THE CITY OF FAIRFAX, LINN COUNTY, IOWA WHEREAS, the City Council of Fairfax, Iowa has contracted with Rathje Construction Co. for the Phase 4 Water Main and Vanderbilt Street Reconstruction Project within the corporate limits; and WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of Change Order#2 as described below: ORIGINAL CONTRACT AMOUNT: $ 752,644.32 ORIGINAL CONTRACT COMPLETION DATE: 85 Working Days ITEM #1: Add 20 SY of Aggregate Driveway Removal (Line Item 10) to the Project Quantities ITEM#2: Add 180 Lf of 8" Dia. PVC Water Main (C-900) (Line Item 43)to the Project Quantities ITEM #3: Add two (2) Water Services (Line Item 46) to the Project Quantities ITEM #4: Add two (2)Water Services (Line Item 48) to the Project Quantities ITEM #5: Additional Disinfection and Hydrostatic Testing (Line Item 50) ITEM #6: Add 20 SY of Aggregate Surfacing for Driveways (Line Item 64)to the Project Quantities ITEM #7 Connections to Existing Water Main (2 Additional Occurrences) ITEM #8 Additional Water Main Fittings (45° MJ Bends) ITEM #9 Add three (3)Working Days to the total project time to allow for completion of this portion of the work During water main installation along Vanderbilt Street, it was discovered that the existing water main, previously believed to be 8" in diameter, was actually 4" water main. Potholing was completed to confirm that this 4" section runs from station 4+00 to the original end of water main replacement location at station 5+80. Water main south of this section was confirmed to be 8" PVC. This change order is for continuing the new 8" PVC water main from the original end location of station 5+80 until the anticipated tie in location near station 4+00. Additional work will include the installation of 4 additional water services, and the removal and replacement of an aggregate driveway disturbed to trench in this water main. An allowance of three (3) additional working days will be granted to the Contractor to perform this work, which will apply to any milestone, incentive, disincentive, or liquidated damages payments.
