FOR THE 2016 SANITARY SEWER INTERCEPTOR MAIN EXTENSION PROJECT IN THE CITY OF FAIRFAX, LINN COUNTY,IOWA WHEREAS, the City Council of Fairfax, Iowa has contracted with Cornerstone Excavating, Inc. for the 2016 Sanitary Sewer Interceptor Main Extension Project within the corporate limits; and WHEREAS,Hall and Hall Engineers, Inc., has recommended approval of Change Order#1 as described below: ORIGINAL CONTRACT AMOUNT: $ 902,615.30 ORIGINAL CONTRACT COMPLETION DATE: 65 Working Days ITEM #1: Remove Line Item 4, "Install Temporary Walkways (10'Wide Temporary Trail)" In its entirety from the Project quantities ITEM #2: Remove 193 SY of"Removal of Existing Sidewalk(Existing Trail and Temporary Trail)" (Line item 11)from the Project Quantities ITEM #3: Remove 37 LF of"Removal of Existing Storm Sewer and Culvert Pipe (Includes F.E.S.)All Pipe is 24" Dia. or Smaller" (Line Item 13)from the Project Quantities ITEM #4: Remove Line Item 35, "Storm Sewer Main, RCP Class III (24" Dia.)" in its entirety from the Project Quantities ITEM #5: Add 10 LF of"Casing Pipe—Boring and Jacking" (Line Item 20)to the Project Quantities AT A REDUCED UNIT PRICE During Drilling and Boring Setup by the Contractor, it became evident that there was not sufficient room to provide a safe temporary trail for use during boring operations under Highway 151. The adjustments of quantities outlined in Items 1 and 2 reflect the negation of the installation of temporary trail, and its subsequent removal from the project scope. The Contractor requested that the existing 24" Diameter RCP Storm Sewer near Station 3+50 of the proposed sanitary sewer alignment be allowed to remain in place during boring operations, and that the sanitary sewer casing pipe be extended beyond the limits of this pipe an additional 10 LF of casing, for the same cost as the total of Items 3 and 4 above. The City's Engineering Representatives determined that this is acceptable because the storm sewer pipe remaining in place will allow any storm water to bypass the bore pit and trench during construction, and the additional casing will reduce the risk of any pressure on form the storm sewer pipe onto the proposed sanitary below. If project conditions dictate that the storm sewer pipe still needs to be removed due to settling 1 damage, the Contractor will perform this work at no additional cost to the City. • Line Item 4, "Install Temporary Walkways (10' Wide Temporary Trail)": 174 LF @ $116.00 1 LF = ($20,184.00) • Line item 11, "Removal of Existing Sidewalk (Existing Trail and Temporary Trail)": 193 SY @$4.001 SY = ($722,00) • Line Item 13, "Removal of Existing Storm Sewer and Culvert Pipe (Includes F.E.S.)All Pipe is 24" Dia. or Smaller 37 LF @$10.001 LF = ($370.00) • Line Item 35, "Storm Sewer Main, RCP Class III (24" Dia.)": 37 LF @ $56.001 LF = ($2,072.00) • Line Item 20, Casing Pipe — Boring and Jacking": 10 LF @ $244.20 EA= $2,442.00 There will be No Additional Working Days allotted for these work changes. The total adjusted Change Order 1 amount is ($20,906,00). Original Contract Amount: $ 902,615.30 C.O. #1- --_ ($20,906.00) Revised Contract Amount: $881,709.30 NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of Fairfax, Iowa, to approve Change Order No. 1 from Cornerstone Excavating, Inc. for the 2016 Sanitary Sewer Interceptor Main Extension Project for a decrease of $20,906.00 making the contract amount $881,709.30. BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and directed to execute said resolution.
