RESOLUTION TO APPROVE CHANGE ORDER NO. 2 FROM
CORNERSTONE EXCAVATING, INC. FOR THE 2016 SANITARY SEWER
INTERCEPTOR MAIN EXTENSION PROJECT IN THE CITY OF FAIRFAX,
LINN COUNTY, IOWA
WHEREAS, the City Council of Fairfax, Iowa has contracted with Cornerstone
Excavating, Inc. for the 2015 Sanitary Sewer Interceptor Main Extension Project within
the corporate Iii-nits; and
WHEREAS,Hall and Hall Engineers, Inc.,has recoinmended approval of Change
Order 42 as described below:
ITEM#1: Additional Connection to Manhole 6
ITEM#2: 6" Certalok PVC Sanitary Sewer Service Pipe
ITEM#3: Boring of 6" Certalok PVC or Damming of Creek and By-Pass Pumping
ITEM#4: Additional Seeding and Erosion Control Work
ITEM#5: Labor/ Equipment to Install 6" Certalok PVC Pipe
At the City's request, this Change Order is to add a 6" Certalok PVC sanitary sewer service from
Manhole 6 to a lot owned by Travis Schrage on the west side of the creek. The service will be
installed either by boring under the creek, or damming the creek, bypass pumping and open
cutting a trench. This Change Order includes payment for core-drilling a hole into Sanitary
Manhole 6, a boot for the 6" service connection, and installation of 70 LF of Certalok PVC pipe.
The end opposite the manhole will be capped for connection by others at a later time.
Please see the attached Change Order Request form from Cornerstone Excavating, and the
revised Plan Sheet D.03 that outlines these changes.
• Additional Connection to Manhole 6: 1 EA @$575.001 EA= $575.00.
• 6" Certalok PVC Sanitary Sewer Service Pipe: 70 LF @$9.951 LF = $696.50.
• Boring of 6" Certalok PVC or Damming of Creek and By-Pass Pumping: LS Item =
$4,950.00.
• Additional Seeding and Erosion Control Work: LS item = LS Item = $350.00.
• Labor/ Equipment to Install 6" Certalok PVC Pipe: LS Item @ $5000.00.
There will be two (2)Additional Working Days allotted for these work changes. The revised
working day total will be 67 Days.
The total adjusted Change Order 2 amount is($11,571.50).
Original Contract Amount: $ 902,615.30

Revised Contract Amount: $883,280.80
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 2 from Cornerstone Excavating, Inc. for the
2016 Sanitary Sewer Interceptor Main Extension Project for an increase of $11,571.50
making the contract amount $883,280.80.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa,that the Mayor and City Clerk/Treasurer are hereby authorized and directed
to execute said resolution.

Description- Add a 6" PVC sanitary sewer service from MH#6 to a lot on the other side of the creek. This will include either
