RESOLUTION TO APPROVE CHANGE ORDER NO. 1 FROM
RATHJE CONSTRUCTION, CO. FOR THE 2016 80th STREET WATER MAIN
EXTENSION PROJECT IN THE CITY OF FAIRFAX, LINN COUNTY,IOWA
WHEREAS, the City Council of Fairfax, Iowa has contracted with Rathje
Construction Co. for the 2016 80th Street Water Main Extension Project within the
corporate limits; and
WHEREAS,Hall and Hall Engineers, Inc., has recommended approval of Change
Order#1 as described below:

CONTRACT AMOUNT: $ 323,871.00
ORIGINAL CONTRACT
COMPLETION DATE: 30 Working Days
ITEM #1: 1,500 SY of Erosion Control Matting
ITEM #2: (10) 16' long Silt Sock for Ditch Checks
This change order is to add a 12' wide swath of North American Green SC15013N coconut fiber
mat to the ditch and silt socks as erosion control checks every 100' between Robson's Farm
driveway and Highway 151. The ditch in this area is steep enough and conveys enough water
that this additional erosion control during and after seeding is warranted. Prices include supply
and installation of materials.

• Erosion Control Matting— North American Green SC15013N: 1,500 SY @$1.82 I SY=
$2,730.00.
• Silt Socks for Ditch Checks (16' Long): 10 EA @$39.601 EA= $396.00.
One (1) additional Working Day will be added to the count to cover installation time for items
implemented under this Change Order. The revised total allowable working days for this project
will be 31.
The total adjusted Change Order 1 amount is$3,126.00.
Original Contract Amount: $ 323,871.00
C.O. #1 $3,126.00
Revised Contract Amount: $326,997.00
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 1 from Rathje Construction Co. for the 2016
801h Street Water Main Extension Project for an additional $3,126.00 making the contract
amount $326,997.00.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa,that the Mayor and City Clerk/Treasurer are hereby authorized and directed
to execute said resolution.
