RESOLUTION TO APPROVE CHANGE ORDER NO. 3 FROM
CORNERSTONE EXCAVATING, INC. FORT E 2016 SANITARY SEWER
INTERCEPTOR MAIN EXTENSION PROJECT IN THE CITY OF FAIRFAX,
LINN COUNTY, IOWA
WHEREAS, the City Council of Fairfax, Iowa has contracted with Cornerstone
Excavating, Inc. for the 2016 Sanitary Sewer Interceptor Main Extension Project within
the corporate limits; and
WHEREAS,Hall and Hall Engineers, Inc.,has recommended approval of Change
Order#3 as described below:
ITEM #1: Install 0.25 Acres of Flexterra HPG M as erosion protection along Highway
151.
At the City's request, this Change Order is to add additional erosion control measures along the
slope adjacent to Highway 151 from East Cemetery Road south as shown in the attached exhibit.
The Flexterra High Performance Growth Medium acts much in the same way as erosion control
matting to reduce the risk of erosion and rifling along the slope. In this situation, the Flexterra
was cheaper than the equivalent area in matting, and is a viable option to achieve the desired
results in this highly visible area of town. Please see the attached estimate from Stevens Erosion
Control (as modified by Joel Gross on 9/7/2016) and the exhibit outlining the area upon which this
additional erosion area is to be applied.
® Flexterra HPGM: 0.25 Acres @$5,620/Acre (plus 10% Contractor Markup) = $1,545,50.
No additional working days will be allotted for implementation of these changes.
The total adjusted Change Order 3 amount is$1,545.50.
Original Contract Amount: $ 902,615.30

C.O. #2 $11,571.50
G. #3 $1,545.50
Revised Contract Amount: $894,776.30
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 3 from Cornerstone Excavating, Inc. for the
2016 Sanitary Sewer Interceptor Main Extension Project for an increase of $1,545.50
making the contract amount$894,776.30.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa,that the Mayor and City Clerk/Treasurer are hereby authorized and directed
to execute said resolution.
