RESOLUTION TO ACCEPT THE
2016 80Th STREET WATER MAIN EXTENTIO�N PROJECT IN
THE CITY OF FAIRFAX,LINN COUNTY,IOWA
WHEREAS, the City Council of Fairfax, Iowa has contracted with Rathje
Construction Company for the 2016 80ffi Street Water Main Extension Project within the
corporate limits; and
WHEREAS, Hall and Hall Engineers, Inc., has stated that this project has been
completed within substantial conformance to the project plans and specifications and has
recommended,acceptance of this project.
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa,to accept the 2016 801h Street Water Main Extension Project.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa,that the Mayor and City Clerk/Treasurer are hereby authorized and directed
to execute said resolution.

RE: 2016 8011 Street Water Main Extension Project— Recommendation to Accept
I hereby certify that this project has been completed within substantial conformance to the
project plans and specifications, and recommend that the City of Fairfax accept this project.
The final payment application for this project pays all retainage, which is being released with this
final payment application upon completion of the 30 day hold period. The final payment
application also includes a Working Day Incentive payment of$2,500 (2.5 days @ $1,0001 day)
and a No Excuses Completion Date Incentive of$10,000. The amount of the final payment is
$38,414.27.
The final contract amount for this project is $338,524.34, which includes all Change Orders (1
total)and the aforementioned incentives. Along with this memo to accept the project is the final
payment application, balancing change order, project lien waivers, and a copy of the as-built
plan set.
