RESOLUTION TO APPROVE CHANGE ORDER NO. 3 FROM
SCHRADER EXCAVATING AND GRADING CO. FOR THE 2017 80"' STREET
WATER MAIN EXTENSION PROJECT IN THE CITY OF FAIRFAX, LINN
COUNTY, IOWA
WHEREAS, the City Council of Fairfax, Iowa has contracted with Schrader
Excavating and Grading Co. for the 2017 80"'Street Water Main Extension Project within
the corporate limits; and
WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of Change
Order#3 as described below:

CONTRACT AMOUNT: $ 74,290.55
ORIGINAL CONTRACT
COMPLETION DATE: 15 Working Days
ITEM #1: Riser Sections to raise three (3) fire hydrants to adjusted slope grade.
At the request of the City, Schrader Excavating installed an additional riser section on the existing
fire hydrant at the northerly end of the existing 2016 water main extension, and an additional riser
section on each of the hydrants installed under this project to bring them up to meet the new
slope elevations. This change allows for easier use for the Fire Department in the event of
emergency. The total cost of this Change Order includes all additional materials and labor to
complete this addition to the project. Please see the attached Change Order Request Form
provided by Schrader Excavating for additional details.
The total adjusted Change Order 3 amount is $3,455.00
Original Contract Amount: $ 74,290.55

Revised Contract Amount: $77,745.55
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Change Order No. 3 from Schrader Excavating and Grading Co.
for the 2017 8011' Street Water Main Extension Project an additional $3,455.00 to the
contract, making the current contract amount $77,745.55.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa, that the Mayor and City Clerk/Treasurer are hereby authorized and directed
to execute said resolution.
