RESOLUTION TO APPROVE EXTRA WORK ORDER(EWO) NUMBERS 1,2
AND 5 FOR THE U.S. HIGHWAY 151 BRIDGE REPLACEMENT PROJECT IN
THE CITY OF FAIRFAX,LINN COUNTY,IOWA
WHEREAS,the City of Fairfax,Iowa has entered into an agreement with the Iowa
Department of Transportation (IDOT) to participate in the U.S. Highway 151 Bridge
Replacement Project for the extension of the City's Sanitary Sewer System and the City's
Water Main System west of and under U.S. Highway 151, as well as other public
improvements to be constructed as part of this project that are the responsibility of the City
of Fairfax; and
WHEREAS, The Bid Price by Peterson Contractors, Inc. for the City of Fairfax
Participation Items was $208,916.47; and
WHEREAS, Peterson Contractors, Inc. has submitted Three Extra Work Orders
(EWO) as described below:
EWO No. 1 — Changes the proposed City Water Main Extension under U.S.
Highway 151 at Church Street to increase the size of this Water Main and Water Main
casing pipe from an 8"diameter Water Main to a 12"diameter Water Main: Price Increase
of$1,626.90;
EWO No. 2 —Changes to the Sanitary Sewer Alignment under U.S. Highway 151
at Church Street to Increase the future capacity of this sewer main and allow for necessary
future sewer improvements to the existing downstream sanitary sewer system: Price
Increase of$7,184.15;
EWO No. 5 — Additional 6" diameter Water Main pipe between the Water Main
Line along Highway 151 and the two relocated Fire Hydrants near Casey's General Store:
Price Increase of$1,330.56; and
WHEREAS, Hall and Hall Engineers, Inc., has recommended approval of EWO
No's 1, 2, and 3 as described above.

PARTICIPATION AMOUNT: $208,916.47
EWO NO's 1, 2,AND 5
AMOUNTS: $10,141.61
REVISED PARTICIPATION
AMOUNT: $219,058.08
NOW, THEREFORE, BE IT RESOLVED, by the City Council of the City of
Fairfax, Iowa, to approve Extra Work Order No's. 1, 2, and 5 from Peterson Contractors,
Inc. for the U.S. Highway 151 Bridge Replacement Project in the total amount of
$10,141.61, bringing City of Fairfax's participation amount in this project to $219,058.08.
BE IT FURTHER RESOLVED, by the Fairfax City Council of the City of
Fairfax, Iowa,that the Mayor and City Clerk/Treasurer are hereby authorized and directed
to execute said resolution.
